Patient Navigation
The role of the Patient Navigation Team at the Brooklyn Free Clinic is to address the social determinants of health that impact our patients’ well-being and access to care. We strive to support underserved and underinsured patients in navigating complex healthcare and social support systems through patient interactions grounded in empathy, cultural humility, and patient-centered advocacy
Patient Navigators conduct individualized social needs assessments and connect patients with affordable health insurance options and community-based resources throughout Brooklyn and New York City. We support patients in accessing food assistance, housing resources, employment and educational opportunities, immigration support services, and other forms of social assistance that may impact their health outcomes and quality of life. Our goal is to meet patients where they are, and empower them with the information and resources needed to make informed decisions about their health.
In addition to direct patient support, the Patient Navigation Team works collaboratively with other clinic teams to promote continuity of care and address barriers that may prevent patients from accessing medical services. We also maintain and continually expand a comprehensive database of community resources to ensure patients receive up-to-date and accessible support tailored to their individual needs.
The Patient Navigation Team is committed to fostering a collaborative and supportive learning environment for student volunteers. Through training, mentorship, and hands-on patient interactions, volunteers develop practical skills in communication, advocacy, and resource coordination while gaining a deeper understanding of the social factors that shape health and healthcare access.
